A duly filled Visa Application Form, to be submitted within 1 month of the date of travel.

A passport that is valid for at least 6 months from your date of arrival in Malaysia. Note that the passport must mandatorily have at least three blank pages for the visa stamping and approval process. In case, there aren’t enough blank pages remaining, a new passport should be applied for, lest the chances of approval of the visa application will stand close to none.
