U dont need visa for Malaysia in this case unless u intent to go out and see Malaysia too.Since your flight layover is only for 2 hrs,you wont be able to do it.
As far as Bali is concerned it is free for Indians as u get Visa on Arrival.

Well, to reach Spiti in November , first of all you you`ll need to take the Shimla -Kinnaur route since the Manali one closes in winters. so, you will be travelling from Shimla, Narkanda, Rampur (on the way, before you reach Reckong Peo, you can take a small detour to Sangla and chitkul, I STRONG ADVICE IT i.e. if you have the time) Then, going through Reckong Peo, Kalpa , Nako, Gue, Tabo and Kaza. Once you are in Kaza, make sure you go to, Key Monastry , Khibber, Langza, Hikkim and Komic. Spiti is my absolute favourite place ever, I am sure you will love the experience.

Hire a car for 3 days, you mean? Not a good idea. if you are looking for car to go to your hotel, it's easily available. If more than 2 ppl the hire entire cab. will cost 800 bahts and if alone or 2 ppl. Hire a shared limo. cost 150 baht per person.
I recommend you to install Grab , it's a online cab and food delivery service in South Asia(Indonesia,Cambodia,Malaysia,Singapore) and I bet their tarriff are much cheaper than the normal taxis , it's just like Uber and Ola
If it's written twin sharing, you need to bring someone with you. once you redeem the package and give your date of departure. they will come back and give your 2-3 dates for you to select.
