Day 1 - Arrival at Railway Station / airport & Transfer to the hotel. Day 2 - South Goa Sightseeing
Mangueshi Temple, Old Goa Church, Miramar Beach, Dono Paula, Church Square Panaji, and Goa Boat Cruise Day 3 - North Goa Sightseeing Fort Aquada, Sinquerim Beach, Candolim Beach, Calangute Beach, Baga Beach, Anjuna Beach, Vagator Beach. Day 4 - Drop to the railway station/ airport. (Days are depends upon your decision for sightseeing)

I think you should not take any package. you can do it without package under Rs 5000 budget. Take Bus from Haridwar Railway Station to Sonprayag and then take taxi to reach Gaurikund which is about 5Km

Try to reach Lourve as early as possible, it gets overcrowded with the time. if you want to enter it for free go on first Friday of the month after 6PM, and make sure to book it in advance. Fast track/skip the line tickets are also available online, opt for those If you do not want to wait in a long queue.
