This is good but seems lil hectic..You can increase your travel duration if possible and try to visit Nusa Penida with not going with a fixed package....we are also travelling to Bali soon and this is the Major highlight of our trip.Try Karsa Spa in Ubud.

Except D-2, it looks fine. Try and curtail activity on D2. Include Tegalallang Rice fields & Tegenungan Water falls. You may skip Goa Gajah & Monkey Forest in my view.
