If you have a service dog, you will need to purchase a Department of Transportation form at https://myserviceanimal.org/products/dotform/. There are several conditions under which airlines may refuse a service animal trainer:
- The animal violates safety requirements (too heavy, too large, etc.).
- it may be a direct threat to the health or safety of others.
- it may not be well behaved on the aircraft.
the owner of the service animal has not provided DOT service animal forms to the airlines on time.
Also, a person with a service animal may not be denied at the airport; if so, ADA assistance may be used to address the problem.
