If you already went to the height of 3 km earlier, then there is no need of acclimatization. If you have not then I will suggest that when you arrive at Leh, stay there at least for 24 hrs. AMS can happen to some and may not happen to some. Like when I went their for the first time... I had severe headache...but then I was O.K after a day in Leh. So precaution is always better then cure. And yes you can see both part of this region in your said time.

If you have 5 days and 4 nights that's enough to cover a part of Leh as well as Nubra Pangong. Acclimatization is very important if you have never been to a hilly area where Oxygen levels are comparatively low. The best way to tackle this is to have a half day rest at Leh post your arrival and evening take a stroll through the market or palace area. You can plan your visit in such a way that out of the four nights you can plan two nights in Leh and once in Nubra and one at Pangong camps.

August is the time where winter starts slowly. best to carry thermal suits, sweater ,two layered woollen dresses, coolers , hand gloves and basic first aid. Remember constipation and dizziness is a common symptom der, so take the medications for these two. as the climate drastically changes from hot to cold take layered clothes as you will be able to adjust . wear on the shoes that is water proof and would give you some bearable standard as while going to nubra valley you will see so many lakes frozen and would want to get yourself walking there. in case you booked through travel agency, they would provide you oxygen mask when u have acute loss of oxygen. take a balaclava as the air may be dry wind and might give a headache for people who could not withstand cold

No that's doable if you are traveling by car that day you can rest next day local sight seeing, third day to hunder via khardungla, fourth day to Pangong Tso and fifth day return or instead of staying at Pangong Tso you can watch it and return the same day.

If you are directly flying down to leh, then it would be better that you spend at least 24 hours to relax and acclamatize. You can stay in leh and cover near by places by walk.
